You will need to complete and return by email:
1. Application
2. Personal Financial Statement for all shareholders

Now gather the following and submit them with the documents above:
1. The past 3 years financial statements prepared by an external accountant on at least a "review"basis
2. The aged listing of accounts receivable and accounts payable that correspond with your most recent year end
3. This Work on Hand schedule of uncompleted work on hand which should also correspond to your most recent year end financial statement.
4. A recent internally prepared month end financial statement
5. The aged listing of accounts receivable and accounts payable that correspond with that recent month end
6. This Work on Hand schedule which corresponds to that recent month end

Also supply any company brochures or reference letters you may have.
